Replacement for Lost/Stolen Passport


If your passport is lost or stolen, you are required to report the loss/theft to the police station in the area which it occurred.  In order to replace the passport, it will be necessary for you to appear personally with the following docments:

Please note: applications for a passport replacement for a child under the age of 16 require the personal appearance of both parents with the child.


  1. Passport application form DS-11  (all passport applications are available in the ACS waiting room) http://foia.state.gov/FORMS/passport.asp

  2. Lost/Stolen passport statement form DS-64  (all passport applications are available in the ACS waiting room); http://foia.state.gov/FORMS/passport.asp

  3. Copy of the local police report;
  4. Proof of identification (e.g. driver license, military id card, a foreign passport that you may currently hold, Kuwait civil id card);
  5. Proof of U.S. citizenship (e.g. expired passport, original/certified copy of U.S. birth certificate issued by the State Vital Statistics Office, Certificate of Naturalization, Consular Report of Birth Abroad certificate);
  6. 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
  7.  Fees: 100 USD or 30 KD for applicants age 16 and above.  Applicants under age of 16 pay 85 USD or 25.500 KD.  All fees are payable in cash only;
  8. Please note that additional documents may be required depending upon the circumstances of your case.

Impotant:  The parent who is not accompanying the child due to travel must submit a notarized statement of consent form DS-3053 with legible photocopy of his/her signed passport.  http://foia.state.gov/FORMS/passport.asp
