Replacement for a Mutilated/Damaged Passport

Please note: that all below would apply if you are applying for a passport replacement for a child under the age of 16, with the personal appearance of both parents, as well as the child.

- A written statement indicating what happened to the passport;
- Proof of identification (e.g. driver license, military id card, a foreign passport that you may currently hold, Kuwait civil id card);
- Proof of U.S. citizenship (e.g. expired passport, original/certified copy of U.S. birth certificate issued by the State Vital Statistics Office, Certificate of Naturalization, Consular Report of Birth Abroad certificate);
- 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
- Fees: 100 USD or 30 KD for applicants age 16 and above. Applicants under age of 16 pay 85 USD or 25.500 KD. All fees are payable in cash only
- Please note that additional documents may be required depending upon the circumstances of your case.
Impotant: The parent who is not accompanying the child due to travel must submit a notarized statement of consent form DS-3053 with legible photocopy of his/her signed passport. http://foia.state.gov/FORMS/passport.asp
