Replacement for a Mutilated/Damaged Passport



In order to replace a mutilated/damaged passport, it will be necessary for you to appear personally.  To make an appointment for this service please click here.  

Please note: that all below would apply if you are applying for a passport replacement for a child under the age of 16, with the personal appearance of both parents, as well as the child.

  1. Passport application form DS-11  (all passport applications are available in the ACS waiting room) http://foia.state.gov/FORMS/passport.asp

  2. A written statement indicating what happened to the passport;
  3. Proof of identification (e.g. driver license, military id card, a foreign passport that you may currently hold, Kuwait civil id card);
  4. Proof of U.S. citizenship (e.g. expired passport, original/certified copy of U.S. birth certificate issued by the State Vital Statistics Office, Certificate of Naturalization, Consular Report of Birth Abroad certificate);
  5. 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
  6. Fees: 100 USD or 30 KD for applicants age 16 and above.  Applicants under age of 16 pay 85 USD or 25.500 KD.  All fees are payable in cash only
  7. Please note that additional documents may be required depending upon the circumstances of your case.


Impotant:  The parent who is not accompanying the child due to travel must submit a notarized statement of consent form DS-3053 with legible photocopy of his/her signed passport.  http://foia.state.gov/FORMS/passport.asp
