First U.S. Passport for a Minor
If you are applying for the first U.S. passport for a child under the age of 18 who was born overseas, you must first report the child's birth. Please read about how to register a birth abroad.
If you are applying for the first U.S. passport for a child under the age of 16 who was born in the United States, the personal appearance of both parents, as well as the child, is a must with the following documents:
1. Passport application form DS-11 (all passport applications are available in the ACS waiting room); http://foia.state.gov/FORMS/passport.asp
2. Child’s original/certified copy of the U.S. birth certificate issued by the State Vital Statistics Office;
3. Recent identifications for both parents (e.g. passports, driver license, Kuwait civil id card);
4. 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
5. Fees: 85 USD or 25.500 KD payable in cash only;
6. Please note that additional documents may be required depending upon the circumstances of your case.
