Passport Renewal for Minors Under age of 16

Passport renewal application for a child under the age of 16 is received based on a scheduled appointment.  The process requires the personal appearance of both parents, as well as the child along with the supporting documents described below:

Appointments are made online from Sunday to Thursday from 1:00 pm to 3:00 pm. 

Make Appointment! 
  1. Passport application form DS-11  (all passport applications are available in the ACS waiting room); http://travel.state.gov/passport/forms/ds11/ds11_842.html 
  2. Child's recent passport
  3. Child’s original U.S. birth certificate or the Consular Report of Birth Abroad certificate;
  4. Recent identifications for both parents (e.g. passports, driver license, Kuwait civil id card);
  5. 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
  6. Fees: 85 USD or 25.500 KD payable in cash of credit;
  7. Please note that additional documents may be required depending upon the circumstances of your case.
Important:  The parent who is not accompanying the child due to travel must submit a notarized statement of consent form DS-3053 with legible photocopy of his/her signed passport.
