Adding Visa Pages
Adding pages is a no-fee service which takes 24 hours.
This service is provided by appointment. Appointments are available Monday through Thursday from 9:00 am to 11:00 am.
Applicants are required to submit passport form DS-4085 with the valid passport. http://travel.state.gov/passport/forms/forms_847.html
Applicants age 16 and above have the option either to bring their passport personally or send it to the consular section through a third party as long as the form is completed and signed by the adult passport holder.
After the second visa pages insert applicants are required to apply for a passport renewal.
In the case of minors under the age of 16:
The personal appearance of one parent is required with the following documents:
