Adult first time passport applicants

If you are an adult who is applying for his first U.S. passport, you are required to bring the following at the time of application: 

1. Passport application form DS-11  (all passport applications are available in the ACS waiting room) http://foia.state.gov/FORMS/passport.asp
   

2. Proof of identification (e.g. driver license, military id card, a foreign passport that you may currently hold)
   
3. Proof of U.S. citizenship (e.g. original/certified copy of U.S. birth certificate issued by your State’s Vital Statistics Office, a Certificate of Naturalization, or Consular Report of Birth Abroad)
   
4. 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
   
5. Fees: 100 USD or 30 KD payable in cash only.
