Adult First Time Applying for Passport

If you are an adult who is applying for his first U.S. passport, you are requested to appear personally on your appointment day with the following documents:Appointments are made online from Sunday to Thursday from 1:00 pm to 3:00 pm. Make Appointment! 

  1. Passport application form DS-11  (all passport applications are available in the ACS waiting room) http://travel.state.gov/passport/forms/ds11/ds11_842.html Proof of identification (e.g. driver license, military id card, a foreign passport that you may currently hold)
  2. Proof of U.S. citizenship (e.g. original/certified copy of U.S. birth certificate issued by your State’s Vital Statistics Office, a Certificate of Naturalization, or Consular Report of Birth Abroad)
  3. 2 recent (taken within six months) photos size 2”x2” (5cm x 5cm) with a white background. Applicant should be facing the camera directly. No profile or ¾ shots. (The Embassy will reject any photographs that do not meet the specified requirements);
  4. Fees: 100 USD or 30 KD payable in cash or credit.
